Job summary

First Community Health and Care is seeking a Rehabilitation Assistant to join the Intermediate Care team, delivering home-based rehabilitation to patients recently discharged from hospital or at risk of admission. You will work with Physiotherapists, Occupational Therapists, Nurses and other team members to support independence and achieve goals.

Applicants must be drivers with access to a car for business use.

Qualifications

• GCSE standard education or equivalent.
• NVQ level 1/2 or equivalent in health and social care.
• NVQ 3 or CQF diploma level 3 in clinical health care support.

Responsibilities

• The post holder works under nurses and therapists in the community setting.
• Carries out delegated tasks from physiotherapist, occupational therapist and nursing team.
• Promotes patient independence through home-based rehabilitation.
