Job summary

Gloucestershire Health and Care NHS Foundation Trust is seeking Home First Assistants for the West Gloucestershire area. You will support adults in their own homes after hospital discharge, delivering person-centred care and reablement to regain independence and daily living skills.

Join a multidisciplinary team, undertake holistic assessments, and work flexibly across sites in the West Gloucestershire team.

Qualifications

• Experience in home care and reablement is desirable.
• Ability to assess needs and plan person-centred support.
• Experience engaging with families and carers.

Responsibilities

• Support adults in their own homes after hospital discharge.
• Carry out holistic and risk assessments and monitor progress.
• Deliver personal and practical care and enable independence.
• Maintain accurate clinical records and contribute to reviews.
• Collaborate with therapists, nurses and social care colleagues.
