Job summary

Midlands Partnership NHS Foundation Trust seeks an Assistant Therapy Practitioner to join Home First in the South of Staffordshire. You will deliver high quality rehabilitation to adults with diverse conditions, working alongside qualified therapists and autonomously within your scope of practice.

The role involves conducting holistic assessments in patients' homes, progressing SMART goals, and coordinating with MDTs to ensure timely discharges and appropriate onward referrals.

Qualifications

• Under the Assistant Practitioner route with FdSc level 5, you will undertake independent assessments within the home setting.
• You will carry out rehabilitation programmes and progress them under supervision.
• Clinical reasoning and safeguarding are expected in patient care.

Responsibilities

• Carry out and progress rehabilitation programmes set by qualified therapists.
• Complete secondary assessments post-discharge with triage and SMART goal setting.
• Discharge appropriate patients and signpost to communities.
