Job summary

Suffolk County Council is seeking an Independence and Wellbeing Facilitator to help customers maintain independence and wellbeing in Ipswich. You’ll identify pathways, provide guidance, and connect people with support and services.

Role requires strong customer service, decision making, and collaboration with partners. 37-hour week, with flexible working options; salary £29,542 pro rata for part-time, and a mix of fixed-term and permanent posts.

Qualifications

• Strong customer service background with high-quality support.
• Ability to analyse information, solve problems and make informed decisions.
• Excellent teamwork and collaboration to meet shared goals.
• Knowledge of local services and providers to connect customers with support.
• Strong communication and interpersonal skills, including handling vulnerable individuals.
• Ability to build relationships with colleagues and stakeholders.
• Good IT skills and comfort with digital systems.

Responsibilities

• Provide advice and guidance to customers and carers about rights, options, and support while assessing eligibility.
• Work with Suffolk’s Heart of Care approach to positively impact residents.
• Champion the use of Assistive Technology and Digital Care solutions.
• Apply a strengths-based approach within the Signs of Safety framework.
• Manage workload, prioritise tasks, and meet deadlines while supporting team objectives.
• Share knowledge and best practices with colleagues for learning and development.
• Complete assessments and referrals, including carers’ assessments and digital referrals.
