Job summary

Suffolk County Council is hiring a Senior Independence and Wellbeing Practitioner in Ipswich. You will assess care and support needs to promote independence and wellbeing, undertaking Care Act assessments and CHC checklists, and supporting decision-making under the Mental Capacity Act.

Your role and responsibilities

In this role, you will assess and review people's care and support needs, ensuring they receive high-quality, person-centred services that promote independence and wellbeing. You will undertake Care Act assessments, arrange and review care packages, complete Continuing Healthcare (CHC) checklists, and support decision-making in line with the Mental Capacity Act 2005.

Working closely with health professionals, service providers and partner organisations, you will carry out telephone assessments, home visits and attend multi-disciplinary meetings to ensure services remain effective, accessible and deliver positive outcomes for the people we support.

The team

This is an exciting time to join our Learning Disability and Autism Service within Adult and Community Services as we continue to improve outcomes for people across Suffolk. We are investing in the future of our service, expanding our Learning Disability and Autism Teams with additional qualified Social Worker and Occupational Therapist roles, while bringing operational delivery and service development together to drive innovation and transformation.
