Job summary

Suffolk County Council is seeking an Independence and Wellbeing Facilitator to support customers to live independently and access the right services. This role sits with Customer First and the Intervention and Prevention Team in Ipswich, with focus on rights-based guidance and wellbeing outcomes.

You will assess needs, provide information, and connect people to appropriate care pathways while working within the local policy framework.

Responsibilities

• Provide advice and guidance to customers and carers about rights, options and support.

• Apply a strengths-based approach within the Signs of Safety framework.
• Manage workload, prioritise tasks and meet deadlines while contributing to team goals.
• Share knowledge and best practice to support team learning and development.
