Job summary

Suffolk County Council is hiring a Senior Independence and Wellbeing Practitioner in Ipswich. You will assess care and support needs to promote independence and wellbeing, undertaking Care Act assessments and CHC checklists, and supporting decision-making under the Mental Capacity Act.

Responsibilities

• Assess and review care and support needs to promote independence and wellbeing.
• Undertake Care Act assessments, arrange and review care packages, and complete CHC checklists.
• Support decision-making in line with the Mental Capacity Act 2005.
• Collaborate with health professionals, service providers and partner organisations.
• Carry out telephone assessments, home visits and attend multi-disciplinary meetings.
