Job summary

Suffolk County Council is seeking a Reablement Support Worker to join the Home First service across various locations in Suffolk. The role supports customers to stay independent at home, with a strong emphasis on promoting health, wellbeing and person-centred care.

You will require a valid driving licence and access to a car. No prior experience is needed as a full induction and training are provided, plus funded Level 2 Health and Social Care qualifications and ongoing team support.

• No experience required as you will receive full induction and in-depth training.

Responsibilities

• Build and maintain positive relationships with customers and their family carers, promoting health, wellbeing and independence.
• Support individuals through personalised care plans and short-term enablement strategies, offering guidance to help them make informed choices.
• Assist with dignified personal care needs and engage with customers and carers to explore alternative ways of meeting long-term needs.
• Promote services that enable safe, independent living at home, monitor progress against care plans and maintain accurate records for ongoing reviews.
