Job summary

Home Group Limited in Leeds is hiring a Reablement Worker (CQC) to support customers aged 55+ with frailty or dementia, helping them regain independence and live well at home. You’ll provide 24‑hour support including personal care, moving and handling, and medication assistance, following support plans and reporting concerns.

You’ll need at least a Level 2 in social care or be willing to work towards it; DBS check is required and paid by us.

Qualifications

• A level 2 or above, or equivalent in social care.

Responsibilities

• Provide 24-hour support including moving and handling, personal care and medication support.
• Supporting individuals over the age of 55 with dementia and physical health needs.
• Follow support plans and report concerns with the care team.
• Assist customers with everyday living activities.
