Job summary

Home Group Limited in Scarborough is seeking a Reablement Worker (CQC) to support customers with frailty and dementia, helping them regain independence at home. You will work with the care team to follow plans, record information, and ensure compassionate, person-centred care.

The role requires an Level 2 social care qualification or equivalent, a caring attitude, and knowledge of local communities. A DBS check is provided and benefits include health plans and a generous leave entitlement.

Qualifications

• Level 2 or above in social care or willingness to work towards it.
• Experience in a CQC care setting is preferred.
• Positive, caring can-do attitude with empathy.
• Experience with support plans and concise record keeping.
• Knowledge of local community and networks.

Responsibilities

• Provide 24-hour support to customers.
• Work with care team to follow support plans and report concerns.
• Support customers with daily living activities.
• Allocate time for conversations and wellbeing with customers.
• Use technology to create and update support plans.
