Care Dorset Ltd is seeking a dedicated Community Reablement Worker to provide in-home reablement support and help individuals regain independence. You will work with a multidisciplinary team to deliver high-quality, person-centred care in clients' homes, promoting dignity and choice throughout the care journey.

Role requires NVQ 2/3 in Health & Social Care (or willingness to work towards it), strong communication, organisational skills and a commitment to safeguarding.

Formación

• Experience delivering personal care and reablement in community/homecare.
• NVQ Level 2/3 in Health and Social Care or working towards it.
• Experience assessing client needs.
• Ability to deliver person-centred care respecting preferences and goals.
• Excellent verbal and written communication.
• Strong organisation and time management to manage caseload.
• Understanding of safeguarding adults and anti-discriminatory practice.

Responsabilidades

• Reablement care delivery to clients in their own homes to regain independence.
• Assist with activities of daily living and mobility as directed.
• Develop and review person-centred care plans.
• Collaborate with health professionals to coordinate care.
• Monitor progress and document changes for managers.
• Maintain safeguarding and report concerns per policies.
• Adhere to CQC standards and participate in mandatory training.
