Job summary

Altogether Care is seeking a dedicated Reablement Worker to support clients in their own homes, helping them regain independence and confidence in daily living. You will collaborate with families and professionals to set goals and provide empowering, person-centred care.

We value a caring, proactive attitude and flexibility to travel across the community. Full training is provided, with a welcoming environment and opportunities for growth within our North Devon services.

Qualifications

• Experience in care or support work is welcome but not essential — full training provided.
• Ability to drive and work flexibly across the community in people’s own homes.
• Energetic, compassionate and enthusiastic about helping others.

Responsibilities

• Work collaboratively with clients to set meaningful goals and identify steps to achieve them.
• Support and assist people in developing skills and confidence to live independently.
• Help rebuild skills in personal care and daily activities, supporting independence with dignity and choice.
• Offer physical and emotional support, encouraging participation in daily living tasks.
• Monitor progress and report when individuals achieve independence and are ready to move on.
• Identify and respond to safeguarding concerns in line with policies and procedures.
• Build and maintain positive relationships with clients, families and professionals to ensure person-centred, safe care.
• Maintain accurate records using mobile technology to ensure quality care delivery and outcomes.
