Job summary

Qualifications

• 1+ year experience supporting individuals with complex care needs in home, community or residential settings.

Responsibilities

• Assist with personal care including washing, dressing, grooming and continence care promoting dignity and independence.
• Provide safe moving and handling support using appropriate equipment.
• Support with medication administration following care plans and company policies.
• Carry out clinical interventions after specialist training (e.g., tracheostomy care, ventilation monitoring, PEG feeding) and report concerns.
• Observe and report changes in physical, emotional, or mental wellbeing.
• Maintain accurate care records and safeguarding procedures.
• Collaborate with nurses, therapists and multidisciplinary teams.
• Work flexibly including evenings, weekends and waking nights.
