Job summary

The Methodist Church in Britain seeks a Work Well Work Health Coach to support people with health-related barriers into and back to work. The fixed-term role covers the north of the county with hybrid working and 22.5 hours per week as a possibility.

Visa sponsorship is not available. You will manage a caseload, conduct holistic assessments, develop personalised action plans, and coordinate with GPs, employers and community partners to enable progression and access to services.

Qualifications

• Coaching or counselling qualification or equivalent experience in information, advice and guidance.
• Good level of education including literacy and numeracy.

Responsibilities

• Manage a caseload of individuals referred into Work Well and provide personalised support.
• Carry out holistic assessments and personalised action plans; review progress regularly.
• Provide information, advice and practical support on employment, training, benefits, health and services.
• Deliver person-centred coaching to build confidence, resilience and health literacy.
• Coordinate with GPs, employers and partners to refer and support service users.
• Assist individuals to access local services and navigate health, social care and employment pathways.
• Deliver group sessions, workshops and awareness activities.
• Maintain accurate records, case notes and outcome data.
