Conexus Healthcare CIC is seeking a Work and Health Coach for the WorkWell programme, based at Sandal Castle Medical Centre in Wakefield. You will provide one-to-one support from assessment through action planning to career guidance, delivering face-to-face, online and phone support across home working, local hubs and community venues.

Key requirements include DBS disclosure and a full UK driving licence (or an alternative travel method).

Qualifications

• Willingness to complete Personalised Care Institute accredited training
• Willingness to complete Conexus Healthcare training
• Experience in one-to-one coaching, advising or supporting

Responsabilités

• Carry out a work and health assessment with each participant and agree an action plan
• Provide one-to-one, personalised support to WorkWell participants
• Offer career and employment guidance
• Identify workplace adjustments for participants
• Refer or signpost to health, wellbeing and community services
• Manage own caseload
• Maintain accurate records to meet targets and report outcomes
• Build relationships with GP practices, employers, jobcentres and community groups
• Deliver support face to face, online and by phone from home, hubs and venues
