Job summary

Herefordshire & Worcestershire Training Hub in Worcester is recruiting a WorkWell coach to support people facing health-related barriers to employment. You will work within a PCN or GP Federation as part of a county-wide network, delivering 1:1 coaching and health/employment guidance.

You will assess barriers, plan interventions, and coordinate with multidisciplinary teams and external services to help participants return to work.

Qualifications

• Experience of supporting individuals with health, wellbeing, employment, or social-related challenges.
• Ability to build positive relationships and engage effectively with people from diverse backgrounds.
• Strong communication and interpersonal skills, including active listening and motivational interviewing techniques.
• Experience of managing a caseload and supporting individuals to achieve goals.
• Understanding of barriers to health, wellbeing and employment.
• Ability to work independently, prioritise workload and manage competing demands.
• Good organisational, administration and record-keeping skills.
• Competent IT skills, including Microsoft Office and electronic record systems.
• Commitment to confidentiality, safeguarding, equality, diversity and inclusion.
• Ability to work collaboratively with healthcare professionals, employers, community organisations and other stakeholders.
• Previous experience in Primary Care, NHS, Social Prescribing, Employment Support, or the Voluntary and Community Sector.
• Knowledge of local health, wellbeing, employment and support services.
• Understanding of welfare benefits, employment legislation, or vocational rehabilitation principles.
• Experience of supporting individuals with long-term health conditions, disabilities, or mental health needs.
• Relevant qualification in health, social care, employment support, coaching, counselling, or a related field.
• Experience of delivering group sessions, workshops, or training.
• Understanding of the PCN model and personalised care approach.
• Full driving licence and access to a vehicle for travel across multiple locations (where required).

Responsibilities

• Provide 1:1 coaching and health and employment advice to overcome barriers to employment.
• Assess participants and identify key health barriers to returning to work.
• Coordinate with multidisciplinary in-house support or signpost to external services.
• Develop a WorkWell pathway work plan and manage caseloads.
• Work with individuals returning to work across diverse backgrounds and needs.
• Collaborate with healthcare professionals, employers and community organisations.
