Job summary

Kent Community Health NHS Foundation Trust is seeking a WorkWell Coach to join a pioneering service bridging health and employment. You will deliver tailored one-to-one coaching to people with physical or mental health conditions to move toward, stay in or return to work.

Working face-to-face, online and by telephone within a multidisciplinary team, you will co-produce personalised action plans, explore health and social factors, and help participants build confidence and work-readiness.

Qualifications

• Tailored one-to-one coaching delivery across health and employment contexts.
• Experience in holistic assessments and person-centred planning may be beneficial.

Responsibilities

• Provide tailored one-to-one coaching to a caseload of participants face-to-face, online and by telephone.
• Co-produce personalised action plans addressing health, wellbeing and employability.
• Support participants with goal setting, job searching, CV development and interview preparation, building confidence and resilience.
• Build relationships with employers and partner organisations to support workplace adjustments and return-to-work planning.
• Maintain accurate records, monitor outcomes and contribute to the growth of the service.
