Job summary

Morgan Hunt is recruiting an experienced Hub Navigator to support individuals experiencing homelessness across Newham, East London. You will deliver intensive, person-centred support, helping clients access housing, healthcare and welfare services while coordinating with partner agencies.

The role involves managing a caseload with complex needs, developing tailored support plans, and maintaining accurate records. Full-time, 6-month assignment with occasional weekends.

Qualifications

• Experience supporting people experiencing homelessness, rough sleeping, or multiple disadvantages.
• Experience managing a caseload and delivering person-centred support plans.
• Strong understanding of complex needs (mental health, substance misuse, social exclusion).
• Excellent communication and relationship-building skills.

Responsibilities

• Build and maintain positive, trusting relationships with individuals experiencing homelessness and complex needs.
• Deliver intensive, proactive, and person-centred support.
• Manage a caseload of clients and develop tailored support plans.
• Support individuals to access housing, healthcare, welfare benefits, substance misuse services, and other specialist support.
• Work collaboratively with partner agencies and multidisciplinary teams to coordinate effective support.
• Advocate for clients and help them navigate services and systems.
• Promote independence, resilience, and long-term positive outcomes.
• Maintain accurate records, reports, and case notes in line with organisational requirements.
