Job summary

Change Grow Live is seeking a Housing Support Recovery Coordinator in London to assess and engage service users, coordinating housing-related interventions and treatment access. You will work with Housing Officers to promote tenancy sustainment and navigate care pathways, delivering holistic support in outreach settings.

Qualifications

• Minimum of 12 months experience facilitating tenancy sustainment.
• Experience engaging with individuals from varied backgrounds and needs.
• Ability to manage challenging behaviour in a supportive role.

Responsibilities

• Establish referral pathways for at-risk individuals.
• Provide wrap-around support in emergency/temporary accommodations.
• Prioritise caseloads by crisis severity and needs.
• Attend multi-disciplinary meetings linking health, social care and housing services.
• Collaborate with local authority and health sectors to prevent homelessness.
• Support budgeting, education, training and employment alongside engagement in structured services.
