Job summary

Eliot Community Human Services, Inc. seeks a Housing First Coordinator to join our Community Support Team, providing housing stabilization support to unsheltered adults in transition to housing. We follow a Housing First model prioritizing permanent housing as the first step in recovery and independence.

You will perform person-centered, strengths-based case management to coach, educate, and connect clients with resources for long-term housing, financial independence, and emotional well-being.

Qualifications

• Bachelor’s or Master’s degree in a human services field or social work; preferred.
• Lived experience with substance use disorder with 2 years sobriety is considered; if only a high school diploma/GED with 1 year of relevant experience, also acceptable.

Responsibilities

• Empower residents with autonomy and self-advocacy.
• Conduct initial needs assessment and develop housing plans with residents.
• Collaborate with staff to incorporate resident preferences and choices.
• Support new residents during housing transitions with partner organizations.
• Assist clients in community integration and accessing memberships and supports.
• Provide practical help, advocacy, coordination, and problem-solving for independent living.
• Promote health and wellness by connecting clients with medical and nutritional resources.
• Intervene in crises using trauma-informed de-escalation techniques.
