Job summary

The Single Homeless Project is seeking a Specialist Housing First Worker to join the Redbridge Housing First team. You will provide intensive, personalised support to people with complex needs in order to move into and sustain a home.

Working across housing, health and social care, you will advocate for clients, coordinate wraparound services and promote harm reduction and client-led planning to build lasting stability.

Qualifications

• Good understanding of Housing First principles and practice.
• Experience working with clients experiencing multiple disadvantages.
• Proven ability to achieve measurable outcomes with complex cases.

Responsibilities

• Accommodation: support clients to move into independent housing and collaborate with landlords.
• Assessment of Need: conduct holistic assessments of support needs and vulnerabilities.
• Support and Action planning: translate needs into SMART, outcome-focused plans.
• Risk Assessment and Management: manage client-related risk and safeguarding concerns.
• Information Management: record client contacts accurately and share information with the team.
• Partnership Working: coordinate with partner agencies to provide wraparound services.
• Social Inclusion: support clients towards greater independence in the community.
• Health and Safety: adhere to SHP policies and lone-working procedures.
