The role involves direct clinical work, crisis response, harm reduction counseling, and collaboration with a multidisciplinary team to coordinate housing placement and community-based care.

Qualifikationen

• Master's degree in related fields.
• Experience working with adults with severe and persistent mental illness or homelessness.
• Ability to provide flexible, community-based services focused on teamwork.
• Must have a valid driver's license and reliable transportation.

Aufgaben

• Provides direct clinical support, harm reduction counseling, and crisis response for assigned individuals.
• Provides weekly clinical support to program staff and clients.
• Collaborates with multidisciplinary team to support clients with complex behavioral health needs and housing placement.
• Supports team in housing placement, transition and orientation.
• Responds to crisis/high risk situations with leadership and field personnel.
• Maintains relationships with outside agencies.
• Meets with high risk clients in the field to assess and coordinate care.
