Job summary

Mental Health America of Los Angeles (MHALA) seeks an experienced Personal Service Coordinator/Intensive Case Manager to provide high‑quality intensive case management for clients eligible for permanent housing under the HFH program. You will support housing stabilization, coordinate services, and document care within EHR systems.

The role requires a client-centered approach, strong teamwork, and the ability to navigate subsidies, housing logistics, and health services for individuals facing

Qualifications

• High school diploma or GED with 2 years' experience preferred.
• Bilingual Spanish speaking preferred.
• Proficient in Microsoft Word, Excel, and Outlook.
• Experience with mental health services preferred.
• Flexible schedule including nights, weekends and holidays.

Responsibilities

• Provide case management support to a caseload of 40 members.
• Assist housing stabilization and document completion for subsidies.
• Outreach and engagement with DHS, CES, and HFH partners.
• Conduct intake and enrollment activities for ICMS.
• Assist with move‑in and orientation to housing.
• Help clients access services and develop individual service plans.
• Offer housing location services and tenant education.
• Ensure documentation standards and EHR entries are completed.
• Participate in on‑call crisis prevention rotation.
• Engage in ongoing professional development.
