Job summary

The North Lincolnshire Home First Community Rehabilitation service is seeking a Reablement Assistant to provide time-limited rehabilitation in clients' homes, following therapy plans and collaborating with health professionals.

The role is 25 hours per week on a rolling rota, with shifts between 07:00 and 23:00, 3–4 shifts weekly. Overtime and weekend enhancements available; salary in the £20,812–£21,575 pro rata range.

Qualifications

• Previous care qualification desirable, though not essential.

Responsibilities

• Provide rehabilitation and reablement support following therapy plans in conjunction with other health professionals.
• Assist with transitions and support discharge from hospital (SDEC and A&E) in a timely manner.
• Carry out moving and handling, personal care, and administer medications as needed.
• Signpost to external organisations to promote social inclusion and emotional wellbeing.
• Work within a rolling 25-hour week rota with shifts 07:00–23:00; overtime and weekend enhancements may apply.
