Job summary

South Tyneside Council is recruiting a Reablement Worker to support adults returning home after hospital stays. This part-time role (12 hpw) at Borrowdale House involves person-centred care, coordinating with Therapy and Support Leads and families to help people regain independence.

You will work a 2-week rolling rota, with early shifts 08:00–12:00 and occasional weekend/bank holiday hours, contributing to safe, holistic reablement plans and enabling residents to live well in their own homes.
