Job summary

North Lincolnshire Council is seeking Reablement Assistants to deliver rehabilitation and reablement support at service users’ homes. You will work within the Home First Community Rehabilitation service, following therapy plans and collaborating with health and social care professionals to promote independence.

The role includes moving and handling, personal care, and medication administration, with a rolling 25-hour week rota and occasional overtime.

Qualifications

• When possible, supports rehabilitation to maximise independence.
• Ability to follow therapy plans with health and social care colleagues.
• Experience in community-based care is advantageous.

Responsibilities

• Support with moving and handling of service users.
• Assist with personal care needs.
• Aid rehab and reablement activities.
• Administer medications as directed by care plans.
• Signpost to external organisations for social inclusion and wellbeing.
• Provide crisis response during unforeseen events.
