Job summary

Lincolnshire Community Health Services NHS Trust in Grantham is seeking a Promoting Independence Therapy Assistant to support discharge-to-assess pathways. You will deliver short-term rehabilitation and reablement in patients' homes, promoting independence after hospital discharge.

Working as part of a multidisciplinary team across Lincolnshire, you will assess needs, plan simple interventions, and ensure safe use of equipment while helping patients remain at home.

Qualifications

• Experience in rehabilitation or reablement settings is desirable.
• Able to work in patients' homes and travel across the county.
• Excellent communication with patients, carers and health professionals.

Responsibilities

• Assist in assessments and planning with qualified therapists.
• Provide reablement care in patients' own homes.
• Support environmental assessment within the patients home.
• Collaborate with the multidisciplinary team to promote independence.
