Job summary

North Cumbria Integrated Care NHS Foundation Trust is seeking a Clinical Support Worker for the Virtual Ward team. The role focuses on helping patients stay safely at home, covering daily care, wellbeing checks and guidance on remote monitoring equipment.

Work will be largely unsupervised in patients' homes, hospital sites or community hubs, with supervision by a registered professional. Level 3 qualification and health/social care experience are required.

Qualifications

• Level 3 qualification or equivalent in clinical care duties.
• National Care Certificate or commitment to complete within 12 weeks.
• Numeracy and literacy at Level 2.

Responsibilities

• Deliver monitoring, care and education to patients and carers.
• Support safe Virtual Ward monitoring and daily care needs.
• Carry out clinical observations and escalate changes.
• Support use of remote monitoring equipment and digital tech.
• Participate in remote assessments and MDT meetings.
• Maintain accurate documentation and patient records.
• Work largely unsupervised in homes, hospital sites or hubs.
