Job summary

North Hampshire Urgent Care is seeking compassionate Clinical Support Workers to join our Remote Monitoring Service. This role involves regular telephone and digital contact with patients at home, reviewing monitoring information, and escalating concerns to clinicians when needed.

You will support onboarding onto digital platforms, provide basic health education, and maintain accurate electronic records. The position requires flexibility across weekdays, weekends, and bank holidays, with shifts

Qualifications

• NVQ Level 3 Health and Social Care or equivalent.
• Care Certificate (or willingness to complete).
• Willingness to work flexibly including weekends.

Responsibilities

• Monitor patient data from a digital remote monitoring system.
• Maintain regular telephone and digital contact with patients.
• Support patient onboarding onto the digital platform.
• Identify deviations from normal parameters and escalate concerns promptly.
• Support patients to use monitoring equipment and digital platforms effectively.
• Provide basic health education and self-management support.
• Maintain accurate and contemporaneous records using electronic systems.
• Respond professionally to messages and telephone calls and escalate where needed.
• Work collaboratively with clinicians and other service staff.
• Consider Caldicott Principles in data handling.
