Job summary

North Cumbria Integrated Care NHS Foundation Trust is seeking a Band 6 Clinical Nurse Specialist for Virtual Wards to lead remote monitoring of patients with acute respiratory illness, Frailty, or Heart Failure in their homes.

You will work within a multidisciplinary team to co-ordinate hospital and community care, deliver specialist advice, manage admissions and discharges digitally, and ensure high clinical standards across the virtual ward.

Qualifications

• Degree-level CPD and post-registration qualifications related to chronic disease management.
• Licensing in nursing or physiotherapy as required by role.
• Experience in respiratory, frailty or cardiology care pathways advantageous.

Responsibilities

• Be an integral member of the virtual ward MDT with partnership working.
• Develop specialist knowledge across virtual ward conditions.
• Provide specialist advice, guidance and care to patients and carers.
• Hold delegated responsibility for virtual ward caseload including admissions/discharges via digital tech.
• Assess and treat patients requiring acute level care within defined care pathways.
• Ensure compliance with professional regulations and Trust policies.
• Maintain skills to deliver evidence-based, responsive care.
• Provide clinical supervision and support to team members.
• Support and deputise for other virtual ward staff as needed.
