Job summary

Nottinghamshire Healthcare NHS Foundation Trust is seeking a Senior Community Nurse to join the Virtual Ward in Mid Nottinghamshire and/or Bassetlaw. You will provide hospital-level care at home within a multidisciplinary team, leveraging remote monitoring and digital tools to support safe discharge and avoid admissions.

The role offers autonomy, opportunities to develop advanced assessment skills, and involvement in innovative virtual care models across multiple sites.

Qualifications

• Awareness of NHS Modernisation Agenda.
• Case management for complex patients.
• Knowledge of the nursing 6 C's.
• Post-registration experience.
• Autonomous working experience.
• Acute care & community care experience.
• Team leadership experience.
• Caseload management & triage experience.
• Supervising and appraising staff.

Responsibilities

• Provide care in patients' homes within the Virtual Ward.
• Work with MDT to support safe discharge and prevent admissions.
• Interpret remote monitoring data and escalate as needed.
• Lead discharge planning and contribute to MDT discussions.
• Support junior staff through supervision and guidance.
• Maintain accurate documentation in the electronic patient record system.
• Work across respiratory, frailty and admission avoidance pathways.
