Job summary

Royal Devon University Healthcare NHS Foundation Trust in Exeter is recruiting for an enthusiastic team member to join the Urgent Community Response (UCR) team. You will support people in the community and work closely with clinicians, families, and the voluntary sector.

You will monitor referrals, answer calls, book visits on the rota, and collaborate with acute hospital staff to help prevent admissions and enable safe discharges.

Qualifications

• Proficient IT skills and ability to use health software systems.
• Experience with health IT systems or willingness to learn Care First 6/EPIC.

Responsibilities

• Communicate with clients, families and clinicians to coordinate care.
• Monitor inboxes/phone lines and manage referrals and communications.
• Book visits on the rota for the UCR team, supporting discharge planning.
