Job summary

Royal Devon University Healthcare NHS Foundation Trust is seeking an enthusiastic Health & Social Care Coordinator (Internal Only) to support the Urgent Community Response team in Exeter. The role combines admin excellence, IT proficiency and patient-centred communication to help prevent hospital admissions and coordinate safe discharges.

You will work 37.5 hours over 5 days on a 7-day rota, including weekends and bank holidays, with enhanced hourly payments.

Qualifications

• Excellent IT skills and ability to work on own initiative.
• Good interpersonal and administration skills with strong prioritisation.

Responsibilities

• Monitor inbox and answer phones; ensure referrals and communications are dealt with promptly.
• Accept referrals for care and book visits on the rota for the UCR team in collaboration with acute hospital and clinical staff.
• Work as part of a team and also independently, showing initiative and flexibility.
• Support colleagues in a fast-paced environment and adapt to changes as required.
