Job summary

Royal Devon University Healthcare NHS Foundation Trust is recruiting for an enthusiastic team member to join the Urgent Community Response (UCR) team in Exeter. You will handle referrals, coordinate visits, and work closely with hospital staff to prevent admissions and aid safe discharges.

The role requires strong IT, customer care and prioritisation skills, with the ability to work in a busy, team-oriented environment and adapt to rota patterns including weekends and bank holidays.

Qualifications

• Excellent IT skills and ability to work independently.
• Good interpersonal and administration skills.
• Ability to prioritise in a fast-paced environment.

Responsibilities

• Monitor the inbox and answer phones to manage referrals and communications.
• Book visits on the rota for the UCR team and liaise with hospital/clinical staff.
• Coordinate with families, clinicians and the voluntary sector to prevent hospital admissions and support safe discharges.
