Job summary

The Royal Devon University Healthcare NHS Foundation Trust is seeking an Urgent Community Response Support Worker to join our community teams. You will help people at home during crises, helping prevent hospital admissions and facilitating timely discharges, across Sidmouth, Seaton, Axminster and nearby areas.

We value commitment to learning and development, with a comprehensive induction and National Care Certificate support. Driving and IT literacy are essential for this community role.

Qualifications

• Level 3 Diploma / NVQ 3 in Health and Social Care or equivalent or equivalent experience in community care.
• Good IT skills for electronic recording.
• Valid driving license is essential.

Responsibilities

• Provide home-based urgent care and support to people in crisis to prevent hospital admission.
• Support safe and timely discharges from hospital and facilitate home-based care.
• Work flexibly across 7 days with varied shifts in the community.
