Job summary

Flow Facilitator role at Frimley Health NHS Foundation Trust requires a proactive administrator to support patient flow, discharge planning, and day-to-day unit operations at Heathlands in Bracknell. You will help coordinate timely, safe discharges and maintain clear communication across MDTs, patients and families.

You will build on your NHS administrative experience, work with social care or discharge coordinators, and contribute to reducing length of stay while ensuring accurate documentation

Qualifications

• Strong administrative experience in a healthcare setting.
• Experience supporting discharge planning and patient flow.
• Ability to communicate clearly with MDTs, patients and families.

Responsibilities

• Support patients through admission and discharge to ensure a coordinated experience.
• Work with the MDT to aid efficient discharge planning and maintain clear communication.
• Provide guidance to patients’ families about the discharge process and available support.
• Liaise with social services, therapy teams, and external agencies as needed.
• Manage day-to-day administrative tasks within the unit.
• Identify delays in patient flow and help implement solutions for faster discharges.
• Maintain accurate documentation and high-quality patient-focused service.
