Job summary

HCRG Care Group is seeking a Band 3 Discharge and Flow Coordinator for Swindon Intermediate Care Centre. You will support safe, timely discharges by coordinating with ward staff, social workers and MDT members, ensuring accurate records and clear communication.

You will visit patients on wards, explain discharge plans, attend MDT meetings and maintain up-to-date electronic records across systems while delivering compassionate patient-centered care. 37.5 hours per week.

Qualifications

  • NVQ Level 3 in Care or equivalent experience.
  • Strong IT skills including MS Office, Excel and Teams.
  • Experience in health or social care with MDTs and discharge planning.

Responsibilities

  • Support safe and timely patient discharges by completing administration accurately.
  • Visit patients on wards to update families on discharge plans.
  • Attend MDT meetings and record actions and discharge readiness in systems.
  • Maintain accurate electronic records and referrals prior to discharge.
  • Communicate clearly with patients, families and the MDT and provide information leaflets when needed.
  • Monitor predicted discharge dates and escalate changes as needed.

# Vacancies Discharge and Flow Coordinator - Swindon## Job Introduction**Help patients get home safely and sooner by keeping our wards moving.**We are looking for a motivated and compassionate Band 3 Discharge and Flow Coordinator to join our friendly Discharge and Flow team, based on **Swindon Intermediate Care Centre****.** This is a rewarding opportunity to make a real difference to patients and their families by supporting safe, timely discharges into community services across our BSW Community Hospitals.In this role, you will work closely with nurses, therapists, social workers and other members of the multidisciplinary team to coordinate patient flow and ensure everyone has the right information at the right time. You will play a key role in supporting smooth discharge processes, maintaining accurate records, completing referrals and ensuring clear communication across services.You will also build strong, supportive relationships with patients and their families on the ward, helping them to understand the discharge process and keeping them informed throughout their hospital stay.**Location:** Swindon Intermediate Care Centre**Hours:** 37.5 hours per week#### #### Package DescriptionYou’ll be joining a supportive and collaborative team who work closely together across the BSW community hospitals.
