Job summary

HCRG Care Group in Bath is seeking a Band 4 Discharge and Flow Senior Coordinator to lead safe, timely patient discharges from Royal United Hospital and coordinate the flow across community services. You will line-manage Discharge Coordinators, work with nurses, therapists, social workers and the MDT, maintain records, and monitor capacity to support efficient patient pathways.

The role is based at Royal United Hospital, Bath, with 37.5 weekly hours, and a salary of £27,674–£30,557 FTE plus

Qualifications

• NVQ Level 3 in Care, Business Administration or equivalent.
• Experience in health or social care settings and MDT work.
• Strong IT skills and ability to use electronic systems.

Responsibilities

• Lead Discharge Coordinators to ensure timely, safe discharges.
• Coordinate MDT activities and maintain discharge records.
• Communicate discharge plans and delays to patients and families.
