Flow Facilitator role at Frimley Health NHS Foundation Trust requires a proactive administrator to support patient flow, discharge planning, and day-to-day unit operations at Heathlands in Bracknell. You will help coordinate timely, safe discharges and maintain clear communication across MDTs, patients and families.
You will build on your NHS administrative experience, work with social care or discharge coordinators, and contribute to reducing length of stay while ensuring accurate documentation
We are seeking a highly motivated, proactive, and dynamic individual to join our team as aFlow Facilitatorat Heathlands. This is an exciting opportunity for someone with strong administrative experience who is ready to broaden their role and play a key part in supporting patient flow, discharge planning, and the day-to-day running of the unit.
The Flow Facilitator will contribute significantly to reducing patients’ length of stay by enhancing communication across the multidisciplinary team (MDT), patients, and families. You will support timely, safe, and effective discharges while maintaining oversight of essential administrative processes that ensure the smooth operation of the service.
This is ideal opportunity for someone who already has got previous experience working with therapy team, adult social care or discharge coordinators within NHS or Adult Social Care in the UK.
This position isnot eligiblefor Skilled Worker visa sponsorship. Applicants must already hold theright to work in the UKfor the full duration of the contract.
