Job summary

Brook Street in Enfield is seeking an experienced Care Coordinator to support delivery of high-quality domiciliary care across the Enfield area. The role combines care coordination, client support, field-based duties and participation in an on-call rota.

You will coordinate staff rotas, conduct assessments and develop care plans, travel to client sites, and ensure safeguarding and compliance. A full UK driving licence and access to a vehicle are essential; on-call cover and mileage are part of

Qualifications

• Experience coordinating domiciliary care services.
• Holds a full UK driving licence and has access to a vehicle.
• Experience undertaking assessments, reviews, and care plan development.
• Strong communication and organisational skills.

Responsibilities

• Coordinate and manage care staff rotas on a daily, weekly, and monthly basis.
• Ensure all care calls are covered and communicated effectively.
• Conduct new client assessments, reviews, and risk assessments.
• Develop, implement, and monitor care plans.
• Carry out spot checks and quality assurance visits.
• Audit care records and daily notes to ensure compliance and high standards of care.
• Provide support, supervision, induction, and guidance to care staff.
• Respond to client, family, and staff enquiries by phone and email.
• Participate in the on-call rota and provide detailed handovers.
• Support safeguarding, incident reporting, and service improvement initiatives.
