Excellence Healthcare Services Ltd is seeking an experienced Care Coordinator to join our domiciliary care team. The role involves coordinating rotas, ensuring safe and effective care visits, and providing operational support within a busy office-based setting.

The Care Coordinator must be prepared to provide emergency care call cover when required and participate in the mandatory on-call rota, with a company vehicle available for authorised duties.

Qualifications

• Experience in domiciliary/home care.
• Care coordination/rostering experience.
• Full UK driving licence.
• Female candidate due to client preference.
• Highly organized, able to manage multiple priorities.
• Calm under pressure with good problem solving.
• Strong communication with service users and families.
• IT skills & rostering systems.
• Strong written and administrative skills.
• Willing to provide emergency care call cover.
• Willing to participate in on-call rota.
• Right to work in the UK without sponsorship.
• Willing to undertake enhanced DBS check.

Responsibilities

• Prepare, maintain and manage care worker rotas.
• Allocate care workers according to service user needs, location, and skills.
• Ensure scheduled visits have appropriate staffing.
• Monitor rota and respond to changes.
• Manage staff sickness, absence and short-notice changes.
• Arrange replacement cover promptly.
• Minimise unnecessary changes and promote continuity of care.
• Coordinate annual leave and staff availability.
• Communicate rota changes to staff and families.
• Support mobilisation and scheduling of new care packages.
• Work with management on capacity for new packages.
• Monitor care delivery using electronic systems.
• Identify late or unconfirmed visits and act promptly.
• Escalate concerns to the manager.
• Maintain records of changes, incidents and actions.
