Qualifikationen

• Experience coordinating domiciliary care services.
• Full UK driving licence and access to a vehicle.
• Experience undertaking assessments, reviews and care plan development.
• Strong communication and interpersonal skills.
• IT literate with scheduling systems knowledge.

Aufgaben

• Coordinate and manage care staff rotas on a daily/weekly/monthly basis.
• Ensure all care calls are covered and communicated effectively.
• Conduct new client assessments, reviews, and risk assessments.
• Develop, implement and monitor care plans.
• Carry out spot checks and quality assurance visits.
• Audit care records and daily notes for compliance.
• Provide support and supervision to care staff.
• Respond to client, family and staff enquiries by phone/email.
• Participate in on-call rota with detailed handovers.
• Support safeguarding and incident reporting.
