Leaders in Care is seeking an experienced Care Coordinator for an established Bournemouth homecare provider. You will own rotas, support care teams and help deliver high-quality community care.

The role runs Monday to Friday, 9am-5pm, with on-call duties on a rota basis and a driving licence is essential. • Experience in care coordination or a similar community care role with strong organisational and communication skills and the ability to manage busy rotas effectively.

Responsabilités

• Coordinating care rotas and ensuring all visits are effectively covered
• Supporting and supervising care workers in the community
• Assessing and reviewing individual care plans
• Completing field-based visits with clients and care staff
• Building strong relationships with clients, families and care workers
• Responding to changes in client needs and resolving day-to-day issues
