Job summary

CCH Group is seeking a Care Coordinator to organize and deliver high-quality home care across our community. You will manage rotas, supervise field staff and ensure care plans are accurate and up-to-date, with travel required using your own vehicle and a valid driving license.

We offer leadership opportunities, expert support and recognition through our Extraordinary Care Awards, with a salary of £25,364 plus on-call payments.

Qualifications

• Experience and organisational skills to manage rotas and build strong relationships with care workers.
• Strong understanding of care plans and rotas is preferred; NVQ3 is desirable.

Responsibilities

• Manage care workers and field-based duties to ensure high-quality home care.
• Assess and review individual care plans and supervise, instruct and support staff in the field.
• Travel required; valid driver’s license and access to own vehicle.
