Job summary

Herefordshire Council is seeking a Family Support Worker (Multi‑Disciplinary) to join the Family Help Central Locality. You will support vulnerable children, young people and families across homes, schools and the community, delivering evidence‑based interventions and coordinating multi‑agency support.

You will write assessments, lead reviews, supervise families, and deliver parenting programmes while maintaining accurate records and contributing to professional development within a supportive

Qualifications

• Level 4 qualification in a relevant field with commitment to professional development.
• Experience working with children, young people and families in social care/ Early Help.
• Experience leading on complex caseloads and coordinating multi‑agency support.
• Knowledge of evidence‑based approaches and parenting programmes (e.g., Triple P or Solihull).
• Strong communication and IT skills, and ability to chair multi‑agency meetings.

Responsibilities

• Write Family Help Assessments and lead Family Help review meetings.
• Deliver interventions as part of the Family Help plan and coordinate joined‑up support.
• Safeguard responsibilities, make referrals, and coordinate multi‑agency services.
• Deliver parenting programmes and engage family networks to support education and activities.
• Maintain high‑quality records, reports and data, and contribute to supervision and training.
