Job summary

Herefordshire Council seeks a dedicated Family Support Worker (Multi-Disciplinary) to support vulnerable children, young people and families across various settings. You will assess needs, plan SMART actions, and lead Family Help review meetings as part of the Families First Programme.

You will deliver evidence-based interventions, coordinate referrals, and maintain accurate records, contributing to safeguarding and outcomes across the county.

Qualifications

• Level 4 qualification in a relevant field is essential.
• Experience with children, young people and families, including lone working.
• Ability to manage complex caseloads and coordinate multi-agency support.
• Experience of leading multi-agency meetings and writing assessments/reports.
• Strong safeguarding knowledge and risk management.
• Willingness to travel across the county and work in family homes.
• Experience delivering parenting programmes (e.g. Triple P or Solihull).

Responsibilities

• Write Family Help Assessments and lead Family Help review meetings.
• Deliver interventions as part of the Family Help plan and ensure joined-up family support.
• Coordinate multi-agency referrals and engagement with partner agencies.
• Maintain high-quality records and prepare written/verbal reports.
• Deliver parenting programmes and engage family networks to support education and activities.
