Job summary

Herefordshire Council is seeking a Family Support Worker (Multi-Disciplinary) to support vulnerable children and families. You will assess needs, coordinate services, and deliver evidence-based interventions across homes, schools, and community settings.

You will manage a complex caseload, write assessments, lead review meetings and ensure timely, joined-up support while safeguarding and promoting wellbeing.

Qualifications

• Level 4 qualification in a relevant field
• Experience with children, families and social care
• Ability to manage complex caseloads
• Experience chairing multi-agency meetings
• Knowledge of safeguarding responsibilities

Responsibilities

• Write Family Help Assessments and lead Family Help review meetings
• Deliver interventions as part of the Family Help plan
• Coordinate multi-agency support and make referrals
• Safeguarding responsibilities and risk management
• Deliver parenting programmes and engage family networks
• Travel across the county and work in family homes when required
