Job summary

Herefordshire Council is recruiting a Family Support Worker (Multi-Disciplinary) in Leominster, Herefordshire. The role involves direct work with vulnerable children, young people and families across homes, schools and community settings, delivering evidence‑based, therapeutic interventions to promote resilience and wellbeing.

You will assess needs, coordinate support, write Family Help Assessments, lead reviews and deliver interventions as part of the Family Help plan, with safeguarding

Qualifications

• Relevant frontline experience with children, young people and families.
• Experience coordinating multi‑agency support and complex caseloads.
• Ability to use motivational interviewing and solution‑focused practice.

Responsibilities

• Write Family Help Assessments and lead Family Help review meetings.
• Coordinate multi‑agency referrals and support plans.
• Deliver parenting programmes and engage family networks.
• Maintain high quality recording, reporting and data management.
