A National Health Service in the UK seeks a motivated Practice Nurse to deliver care to patients, focusing on managing long term conditions like Asthma and COPD. The role requires clinical skills, effective communication, and experience in primary care. We are looking for a highly motivated, friendly and organised Practice Nurse with a keen eye for detail who wants to make a difference to patients, and who is qualified in managing Long Term conditions such as Asthma, COPD and Diabetes. We are looking for someone for 20-25 hours per week, who will, at all times, deliver the high level of evidence based individualcare to the Practice population. The post holder will have the responsibility forthe provision of nursing services related to General Practice which willinclude participating in the assessment of patient care which will include long term conditions management, treatment,preventative care, screening and patient education. The post holder will workat times without supervision.
The post holder will be able to practice a range of clinicalskills developed through experience and training according to Practiceprotocol.
The post holder will ensure clinical practice is safe andeffective and remains within boundaries of competence and to acknowledgelimitations including:
Wehave a fantastic team here at Haslingden which comprises of 2 GP partners, 1salaried GP, 3 x regular locum GP's, 1 ANP, Practice Nurses, plus numerousadditional roles staff including a First Contact Physio, Trainee AssociatePsychological Practitioner, Physician Associate, Pharmacist, PharmacyTechnician and Health and Wellbeing Coach.
Ouradmin team comprises of a Practice Manager and deputy together with manysupportive staff including reception.

This role requires consolidation ofspecialist knowledge and skills in general practice nursing demonstrating adepth of knowledge, understanding and competence that supports evidencedinformed, complex, autonomous and independent decision-making and care ingeneral practice and related settings. This role will require personalresilience, management, clinical leadership and supervision and mentorship ofothers in the general practice nursing team and providing an effective learningenvironment for staff and students in the wider team. The role will require aninnovative approach in supporting and developing new models and strategies forservice delivery, usually incorporating inter-professional and inter-agencyapproaches to monitor and improve care. General Practice Nurses deliver care tothe practice population, but also need to have an understanding of the public health profile and population needs in order to be proactive in ensuringservices are, as far as possible, matched to need. The postholder will, at all times, deliver the high level of evidence based individualcare to the Practice population. The post holder have the responsibility forthe provision of nursing services related to General Practice which willinclude participating in the assessment of patient care to include treatment,preventative care, screening and patient education. Thepost holder will develop and maintain an in-house training programme reportingto the Nurse Team Leader
PersonalDevelopment plans will be encouraged and supported by the Practice. It is theindividuals responsibility to remain up to date with recent developments
Aswell as the nursing team there is a need to work closely with reception andadmin to ensure the smooth running of the Practice
Anyproblems should be reported initially to the Nurse Team Leader
Thereis also the need to establish and maintain good liaison with other surgeriesand agencies including secondary care
Thepost holder may have access to confidential information relating to patientsand their carers, Practice staff and other healthcare workers; they may alsohave access to information relating to the Practice such as a businessorganisation. All such information from any source should be regarded asstrictly confidential.
Informationrelating to patients, carers, colleagues and other healthcare workers may onlybe divulged to authorised persons in accordance with the Practice policies andprocedures relating to confidentiality and the protection of personal andsensitive data
