Croydon Health Services NHS Trust is seeking an Elective Patient Pathway Navigator to ensure accurate RTT pathway data and timely management of waiting lists. Elective Patient Pathway Navigator (EPPN) is responsible for ensuring that the timeline data for patients on the waiting lists (RTT and non RTT, Planned and open referrals) is accurate and appropriately reflects the patients' current position on the pathway. The primary aim will be to ensure that the service provides an effective service for patients whilst responding to the needs of the Trust for RTT management.
The post-holder will be responsible for monitoring and validating the admitted and non-admitted Patient Tracking List, for reporting the RTT position on a weekly basis and taking proactive action to prevent individual breaches and to support the delivery of action plans to improve the RTT performance. They will support the Elective Pathways Operations Manager in the provision of support to the admin and clerical teams and medical teams, for example by training on RTT issues, outcome forms and rules around RTT management and the Trust's Access Policy. The post involves liaison with patients, parents, consultants, junior medical staff, nurses, managers and administrative staff across PODs. The post holder will therefore need to be a highly effective communicator, be pro-active and able to work fairly autonomously and to have a good knowledge of RTT management good practice.

The post holder will work as part of the Endoscopy admissions team to provide a high-quality, patientfocused admissions and booking service. The role involves coordinating elective admission pathways from referral to treatment, ensuring patients are booked in line with Trust Access Policy and national waiting time standards.
This is a busy and varied role requiring excellent organisational skills, sustained concentration, and the ability to manage competing priorities in a frequently interrupted environment. The post holder will regularly handle sensitive and confidential information and must communicate effectively and empathetically with patients, relatives, clinicians and a wide range of internal and external stakeholders.
The Elective Admissions Coordinator will work with a degree of autonomy, using initiative and judgement to manage waiting lists, resolve booking issues and elevate concerns appropriately. Close liaison with consultants, Cancer Services, Pathway Coordinators, the endoscopy team and support services is essential to ensure efficient use of endoscopy capacity and a positive patient experience.
